The Self Directed Support Process

Self Directed Support is an outcome we want to achieve for everyone. However to get to that outcome a process needs to be put in place to make that happen. The process of getting to a point where someone can direct their own support generally has a number of stages in it and this is what is meant by the Self Directed Support process. These stages can vary slighly between local authorities but the principles remain the same.

Stage 1  Self evaluation

Stage 2  Resource allocation

Stage 3  Outcome based support planning

Stage 4  Agree the plan and money management option

Stage 5  Live your life

Stage 6  Monitor the money and review the outcome

These stages are explained in more detail over the next few pages.
